Who We Are

  • Initial Family Assessment (IFA) - We recognize that not everyone begins his/her journey from the same starting point. An IFA gathers information to tell the family's "story." The narrative that is developed can be utilized by the family in working with providers thereby reducing the number of times that they need to provide the same information. The IFA provides information on the family's resources, priorities, concerns, and unique characteristics in order to support the family in identifying the programs and services that would be most beneficial to a special needs child and his/her family.

  • Family Care Plan (FCP) - Information from the IFA and specialty evaluations are used to guide the family and the Solutions CEDARR team in the development of a comprehensive Family Care Plan. Parent(s)/Guardian(s) are included in every stage of the process since they are most constant throughout the child's life. The goal of the FCP is a strong family voice in developing a plan that includes both formal services and informal supports that "wraparound" the family. The FCP includes all family members and can be reviewed and adjusted at anytime. Usually it is rewritten at least every six months for as long as the family is participating in Solutions CEDARR.

  • Family Care Coordination Assistance (FCCA) - FCCA is an optional follow-up service offered to families after developing the Family Care Plan. Coordination is an active and ongoing process of supporting families. During the six months of an active FCP, staff will follow-up with families, direct service providers, and others involved to ensure that services provided to families are those identified in the FCP and that services are meeting the needs of each family.

  • Enhanced Services - Enhanced Services are intended to respond to the immediate needs of children and families. These may be emergency in nature or may support a diagnostic process in developing the IFA.
